Advance Directives

When it comes to your health and health care, what matters to you? By answering this question and sharing the answer with your family and health care providers, you can not only make decisions for yourself that match your values but you can also play a more active role in your own health care. An important aspect of knowing what matters to you is deciding who you want to speak for you if you are unable to speak for yourself due to illness. Below are links to Health Care Power of Attorney documents in multiple languages that you can complete to be sure that what matters to you is fulfilled no matter what your health condition is.

In addition to a Health Care Power of Attorney, there are several additional Advance Directive documents that maybe helpful to you. Advance directives are legal documents that allow you to make decisions about end of life ahead of time so that you can tell your wishes to family, friends and health care professionals to make sure your wishes are fulfilled. No one can predict when a serious illness or accident might happen. When an unexpected crisis occurs, advance directives allow you to control the things you can control.

If you have questions or would like assistance with these forms, please contact your primary care doctor’s office.

Once you complete the forms, please make copies for you to keep in a safe place. When you go to your doctor’s office, make sure that the forms get scanned into your medical record, in addition to sharing a copy with the person you assign as your Power of Attorney.
